The International Society of Minimally Invasive and Virtual Surgery (ISMIVS, previously known as International Society of Minimally Invasive and Noninvasive Medicine) was founded in July 2013 at the gala of the 1st Yangtze International Summit of Minimally Invasive and Noninvasive Medicine in Chongqing, China. ISMIVS envisions to accelerating the progress in minimally invasive and noninvasive medicine by promoting research, education, communication, and international collaboration.
国际微无创医学会(ISMIVS)是在 2013 年 7 月在中国重庆召开的第一届国际微无创医学长江高峰论坛上宣布成立的,旨在通过学术研究、教育、交流以及国际合作加快微无创医学的发展。

The Personality in the Medical Education World 医学教育界的大人物
From the 2020 July issue of our ISMIVS Newsletter, we would like to introduce some pioneers in the field of minimally-invasive and noninvasive medicine. They are not only prestigious in their field but also willing to share their experience and ideas with younger doctors, hence making a great contribution to medical education. This issue features Prof. Tsung-Ho Ying from Chung Shan Medical University, who is a specialist in the field of gynecology and obstetrics.
从 2020 年 7 月刊开始,国际微无创医学会将为大家介绍微无创医学领域的一些领军人物。他们不仅在各自的领域享负盛名,而且非常乐意与年轻医生分享他们的经验、理念和想法,从而为医学教育做出了巨大的贡献。本期月刊要介绍的是中山医科大学的妇产科专家应宗和教授。

Prof. Tsung-Ho Ying is an experienced specialist in Obstetrics and Gynecology. He acquired his M.D. and PhD degrees from College of Medicine, Chung Shan Medical University, and has been appointed as Associate Professor of School of Medicine at Chung Shan Medical School since 2012. He is now the Vice Superintendent of Chung Shan Medical University Hospital.
His expertise is in obstetrics and gynecology, biochemistry, perinatal medicine, medical management, ultrasound, high-risk pregnancy, female urology, endoscopic diagnosis and treatment, and medical practice management. Dedicated to treating patients with less harm, he began to learn high-intensity focused ultrasound (HIFU) and successfully completed his first HIFU surgery in 2014. Since then, Prof. Ying has been involved in numerous global symposiums to share his clinical experience performing HIFU and promote this innovative noninvasive technique as main speaker.
他专长于妇产科学、生物化学、周产期医学、医疗经营管理学、超声波学、高危妊娠、妇女泌尿学、内视镜诊断治疗和医务经营管理。在此基础上,秉持着让病人受伤害更小的理念,他开始学习应用高强度聚焦超声(HIFU),并在2014年成功完成了他的第一台 HIFU 手术。此后,应教授受邀作为主讲师参与了多个 HIFU 主题的国际研讨会,分享其HIFU临床经验,推广这一创新无创技术。
Prof. Ying is an active board member in medical societies, including the Taiwan Association for Minimally Invasive Gynecology, Taiwan Society of Perinatology, and Taiwan Urogynecology Association. He is also on the editorial board of Taiwanese Journal of Obstetrics & Gynecology.
同时,应教授是台湾妇产科内视镜微创医学会(Taiwan Association for Minimally Invasive Gynecology)、台湾周产期学会(Taiwan Society of Perinatology)和台湾妇女泌尿暨骨盆医学会(Taiwan Urogynecology Association)的常任成员。他也是台湾妇产科杂志的编委会成员。

Under much anticipation, the 2022 APAGE-OGSM-ISMIVS Meeting for Minimally Invasive and Noninvasive Surgery was successfully held in Melaka, Malaysia on November 13, 2022. The meeting, co-organized by Gynecologic Endoscopy and Minimally Invasive Therapy (APAGE), Obstetrical & Gynecology Society of Malaysia (OGSM) and International Society of Minimally Invasive and Virtual Surgery (ISMIVS), addressed the topic of Laparoscopic Surgery and High-Intensity Focused Ultrasound (HIFU) therapy. Co-chaired by Dr Andy Tan (Singapore General Hospital) and Prof. Chyi-Long Lee (Chairman and Board of Trustees of APAGE), the meeting was joined by experts and scholars from all over the world who shared their experience and insights on minimally invasive and non-invasive treatment.
万众期待下,2022 年国际微无创医学会(ISMIVS)与亚太妇产科内视镜暨微创治疗医学会(APAGE)马来西亚妇产科学会(OGSM)联合会议暨 2022 年马来西亚微无创手术大会,于2022年11月13日在马六甲顺利召开。本次会议由APAGE,OGSM 和 ISMIVS 共同举办,探讨腹腔镜手术与超强度聚焦超声治疗相关主题。来自新加坡综合医院的 Andy Tan 医生与 APAGE 董事会主席李奇龙教授共同担任会议主席,全球各地的专家学者在会议上分享了微无创医学的相关经验与思考。
Prof. Feng Wu from Oxford University addressed the topic of HIFU ablation and antitumor immune response. The best scenario in cancer therapy is killing all tumor cells including primary and metastatic sites without causing any harm to healthy cells in the body. As non-selective radiotherapy and chemotherapy have strong side effects, professor Wu raised the possibility of manipulating an immune response against tumor cells but not normal cells, and list studies on the immune response after HIFU treatment. In conclusion, he noted that HIFU Therapy not only induces the death of all localized tumor cells without damage to healthy cells but also activates systemic antitumor immunity.
牛津大学的伍烽教授探讨了 HIFU 消融与抗肿瘤免疫反应。癌症治疗的最优方案为消灭原发及转移部位的所有癌细胞,且不对体内的健康细胞造成任何伤害。由于放疗和化疗都具有非选择性,且副作用严重,伍烽教授提出通过操纵免疫反应针对抗癌细胞,正常细胞就不会受到影响,并列举了 HIFU 治疗激活免疫反应的相关研究。他在结论中提到,HIFU 治疗不仅能够杀死局部肿瘤细胞且不破坏健康细胞,还能够激活系统的抗肿瘤免疫。
Focusing on the application of HIFU technology, Prof Mohamed Hamed (from Cairo University) gave a lecture on the Management of liver tumor & HIFU applications in other extended indications. Professor Hamed listed the advantages of HIFU technology, which is non-invasive and re-treatable with high safety profile and no ionizing radiation, and shared related cases and studies.
Mohamed Hamed 教授(开罗大学)聚焦 HIFU 技术的应用,讨论了肝癌的管理与 HIFU 在其它适应症中的应用。他首先列举了 HIFU 技术的优点,包括具有非侵入性、可以重复治疗、安全性高且没有电离辐射,然后分享了相关病例和研究。马来西亚的 Selva 医生也详述了如何将 HIFU 技术应用于子宫腺肌症导致的不孕。
The president of the International Society of Minimally Invasive and Virtual Surgery (ISMIVS), Prof. David Cranston from Oxford University Hospitals shared their experience on the clinical treatments and trials of HIFU therapy. He noted the results showed that there was an increasing number of studies showing the effects of HIFU treatment as good as other localized treatments and the side effects were less.
ISMIVS 主席,来自牛津大学医院的 David Cranston 教授分享了他们关于HIFU的临床治疗与实验的相关经验。他提到研究结果显示,越来越多的研究证明HIFU治疗的效果与其它局部疗法同样好,同时副作用更小。
Professor Jordi Rodriguez and Dr S. Selva focused on the topic of HIFU and fertility. Prof. Jordi Rodriguez shared their studies on the pregnancy outcome after HIFU, which was a single-centre result from Spain. He mentioned at the end of the speech that USgHIFU therapy was an effective and safe treatment for uterine fibroids and it seemed to be the first therapeutic option in women with uterine fibroids and gestational desire. Dr. S. Selva also gave a detailed presentation on how could HIFU help with infertility with adenomyosis.

Dr. Kim Tae See introduced his experience on how to enhance HIFU sonication. He shared some measures to improve the safety of the treatment. By reducing sonication power, exposure time and air bubble, the complication rate could be lowered. He also listed other methods such as protecting nerve by making a distance from the sacral area.
金泰熙医生介绍了如何增强 HIFU 的疗效,并且分享了一些提高治疗安全性的措施,如通过降低声功率,缩短辐照时间,减少气泡,能够有效降低并发症的概率;控制治疗区域与骶骨区之间的距离可以保护神经。
Dr He Min from the Chongqing Haifu Hospital also introduced the application of HIFU technology and focused on the training experience in China. On the same day launched the opening ceremony of the 2022 International Training Workshop on Focused Ultrasound Therapy for Tumors, which gathers momentum for the development of HIFU technology.
重庆海扶医院的何敏医生也介绍了 HIFU 技术的应用和医生培训经验。当天,2022 年聚焦超声肿瘤治疗培训班正式开幕,为HFIU技术的进一步发展助力。

HIFU Special Issue 高强度聚焦超声特刊

We are delighted to announce that the International Journal of Hyperthermia, the HIFU Special Issue (i.e. Volume 38, 2021), which was partially supported by ISMIVS, was officially released in September. The Special Issue includes 15 articles written and reviewed by world-renowned scholars and pioneers in minimally invasive and noninvasive medicine, elaborating on the status and prospects of the clinical applications of high intensity focused ultrasound on tumours that occurred in specific organs. We'll share the 15 articles included in the upcoming newsletters, stay tuned!
Is high-intensity focused ultrasound (HIFU) an option for neoadjuvant therapy for borderline resectable pancreatic cancer patients? - a systematic review

This article is a systematic review of the data about the resection rate after HIFU ablation in patients with borderline resectable pancreatic cancer (BRPC) and the impact of this technique on the oncological results.

From the included studies 97 patients underwent neoadjuvant HIFU ablation ± chemotherapy. Thirty-four patients reached resection (35.1%). In two patients, residual tumor (R) classification was not reported. The R0 resection rate in all reported patients is 30.5% (29/95). R1 resection rate is 3.2% (3/95).

HIFU is found to be safe and feasible in locally advanced and metastatic pancreatic cancer with proven downstaging and downsizing effects. Further research on the role of HIFU ablation as a neoadjuvant treatment for borderline resectable pancreatic cancer is needed.

Quality of life in patients undergoing minimally invasive surgery

To assess the quality of life of patients undergoing minimally invasive surgery in a gynecologic oncology department over time and to establish the relationship between patient and treatment variables. The specialists from Colombia published a paper containing a total of 158 patients. Belonging to a higher socioeconomic stratum (3 vs 1) had a positive impact on the global quality of life in patients undergoing surgery with an increase of 7.6 points (p=0.011). Also, a lower Charlson Index had a positive impact of 0.393 points for the physical component of quality of life (p=0.031).

The results indicated that a lower Charlson Index, having a partner, or having higher socioeconomic status are all associated with a higher quality of life for patients undergoing minimally invasive surgery.
